The APPLICATION for flood insurance is, understandably, a little more complicated than some of the other property and casualty policy applications as it covers a specific risk on property that must be identified although the range of buildings and property can run the gamut. Because of this need to be precise, this form must be completed only in the correct fashion as there is no room for guessing.

This Application form or a similar form used for WYO companies, must be used for all flood policies except the Preferred Risk Policy. The NFIP Flood Insurance Application form, or a similar form for WYO companies, must be used for all flood insurance policies except the Preferred Risk Policy.

The Application form must be completed accurately as it contains the necessary information required by statutory intent and it provides the instructions in respect to the rating of the different building types.

The Application provides the initial information so that the flood insurance rate can be properly applied to the building, particularly as to whether the building is Post-FIRM construction or Pre-FIRM construction; and provides information as to building occupancy, building type, basement type and elevated building type.

The flood risk Zone must also be determined as well as the elevation of the building.

TYPES OF BUILDINGS

For purposes of the NFIP, distinctions have been made among the following building types:

1. No basement

- One floor

- Split level

- Two or more floors

1. Unfinished basement

- Split level

- Two or more floors

1. Finished basement

- Split level

- Two or more floors

Manufactured (mobile) home, including doublewide, or travel trailer, on foundation

2. Elevated building

- No enclosure

- With enclosure (including crawl space)

An elevated building is a building that has no basement and that has its lowest elevated floor raised above ground level by foundation walls, shear walls, posts, piers, pilings, or columns.

SCHEDULED BUILDING POLICY

1. To obtain a Scheduled Building Policy, an Application must be completed for each building and/or contents for which coverage is requested.

2. For each scheduled building (building and/or contents coverage); the Federal Policy Fee is .00 per building

3. All Flood Insurance Application forms must be completed in accordance with all Flood Insurance Manual rules and the Scheduled Building Policy qualifications.

4. If requesting a Scheduled Building Policy, indicate Building #1, Building #2, etc., in the upper right corner of each Application. Staple together the Applications as a single unit.

Physical Therapy Continuing Education recently interviewed a Hip Surgery Rehab Expert about his process for rehabbing people post FAI Hip surgery.

Here are some of his answers:

Hip Surgery Rehab Expert:That being said he asked the patients to stay at least four to five days following the surgery, come twice a day, really get hands on as far as whats involved with the rehab, have a thorough understanding of that rehab process. The more times that patient can hear and have the rehab performed on them the more theyre gonna remember once they leave.

We couldnt always rely on if the patient was going back somewhere where there was a therapist available or they didnt have the means to go to therapy following this, so we also tried to make the rehab process as much possible for the patient to perform on their own with just some follow-up care as needed.

So, our main hip surgeon, when he came out here he had for the most part the same population and so he adopted that philosophy. He asked that the patients stay at least four to five days, to get in twice a day while theyre out here and we go through their specific program with them.

They get treated twice a day and then once they leave here they have the information to contact us via email, phone call to continue up with either their therapist or once they leave here their therapist or they themselves can follow-up with us.

Because our hips surgeon's practice is just exploding we did bring on an athletic trainer who has allowed us to communicate more openly with these patients and the therapists once they leave here. So, we have one full staff person dedicated purely to communicating with the patients and the therapists once they leave.

Interviewer: Excellent. And then follow-up. Do people actually come back to Vail for reassessment or how does that side of things work?

Hip Surgery Rehab Expert:Yes. What well do is theyll stay here four to five days, and what we found is that a lot of patients will stay a little bit longer. Weve seen more and more people dedicating two weeks out here. Obviously its Vail and theres some expense with spending the days out here and the nights out here, so we understand that, but some patients have made the they put the effort into finding one of the best physicians in the world doing the surgery in the rehab facility they figure they might as well spend a little extra money and stay out here. So, they are staying anywhere from four days to two weeks, three weeks up to some patients staying their whole rehab process.

That being said, even if they leave after four or five days we have them come back at probably about six to eight weeks which is right around the time when theyre starting to get more into their functional activities. Kind of going from phase two into phase three of their rehab. Thats around the time that we ask them to come back, around that six to eight week mark and/or they will come back at around 12 weeks.

Bernadette was diagnosed with breast cancer in Oregon. Her doctor in Oregon told her that she needed surgery as soon as possible. However, since she was not employed and had no insurance, she decided to move back home to Indiana.

During a routine claims review, the insurer sent an inquiry to the Public Health Service in Oregon, which duly reported the earlier findings. Based upon these findings, the insurer canceled the policy based upon a material misrepresentation and refused to pay for any of the medical costs.

BUILDING ELEVATION INFORMATION (SURVEY REQUIRED)

The responsibility of providing a number of elevations based on the building type selected is that of the surveyor, engineer, or architect. From these elevations gathered, the agent is required to determine the lowest floor for rating flood insurance.

The surveyor, engineer or architect should complete Section C, but if it is left blank, the agent should contact the responsible person who completed the form and the insurance company underwriter with any questions.

Section D is the certification of the surveyor, engineer or architect that the information provided by them is correct and their signature and identification number are required—in some states, a seal may be required.

BUILDING ELEVATION INFORMATION, ZONE AO AND ZONE A (WITHOUT BFE)

Section E - The difference between the highest and lowest elevation grade are required. For the designated Zones, the property owner or his representative may completed Sections A, B & E.

PROPERTY OWNER OR REPRESENTATIVE CERTIFICATION

Address and other contact information about the property owner are requested in Section F. The party completing Sections A, B, C and E must complete this section also.

COMMUNITY INFORMATION

The local official authorized to administer the community's floodplain management ordinance may transfer elevation information found on existing documentation—such as from an older elevation certification form or surveyor letterhead—to Section C, and then certify this information by completing Section G. A statement advising FEMA of this transfer of information must be made in the comment section of the EC. Section G may also be used to certify Item E4.

Then the bone-patellar tendon- bone graft really developed and gained as the foremost graft in the 80s, as fixation for this was quite good, the strength of this was quite good, and really there were good techniques involved in using the arthroscope.

Hamstrings were then developed around the same time and utilized in the late 80s, early 90s, into the 90s, and gained favor as this was an alternative graft. The problem however, and some of the early research and literature was that the fixation for the hamstring or soft tissue grafts wasnt quite as good as the patellar tendon. Therefore the clinical results seemed to be behind that of the patellar tendon graft. So then it lost a bit of favor in the 90s maybe into the 2000s.

Then with the development of other fixation devices, it has made a surge back into favor and certainly is a nice alternative graft source and a nice graft for reconstruction of the ACL. It now is thought of, in terms of fixation, as similar to that of patellar tendon and really thought equally in terms of utilization as a graft in many instances. ADVERTISING

For the purpose of this discussion and regulations, "advertising" applies not only to "ads" (which actually is an abbreviation for advertisement...).

REFUNDS

Refunds will be made by the insurance company for the current and one prior year. If refund is requested for a period of more than 2 years, then the NFIP must process the request. The insurer must also include statistical records for each policy term and evidence of premium having been received—accounting details which should be expected.

CANCELLATION REASON CODES

The following reason codes for cancellation/nullification of NFIP policies is a summary of the reasons as most of them are self explanatory. For more details, please refer to the Flood Insurance Manual.

1. Building Sold or Removed. Insured has no more insurable interest, or any other reason that the building is sold or removed. If the building is sold, proof of sale is required. Request must be received within 1 year of sale/removal, accompanied by Bill of Sale, settlement statement, or other such proof of removal or total loss. Up to 2 years pro rata refund

I have been in my current position in my firm now for over three years. I do not find my job challenging enough or if I am honest rewarding enough monetary wise or fulfillment wise. I do not feel excited about coming to work the way I used to only a year ago.

My problem is that my current level of education means I have reached as far as I can go in my particular field. The new “kids on the block” who join our firm straight from university have degrees coming out of their ears. The only thing they lack is experience but this can be accumulated within a couple of years. I have done some research and within two years, the latest young high flyer will have accumulated enough experience to side step me. So in as little as two to three years he could be my boss. Whatever I decide to do, I need to do it now.

I need to get an equivalent degree if not a master’s degree.

This would see me into my well-deserved retirement and ensure the younger management members will remain my subordinates. I have to initiate my continuing education program and fast if I wish to fulfill my ambitious plan in the tight time scale. My continuing education means time away from my desk, attending lectures. The written work is no problem as this can be done in the evening at home or in the office if I need to access files etc.

No matter what type of annuity you purchase, it is subject to a 10 percent IRS penalty for withdrawals of growth of income made prior to age 59 . No penalty is imposed on one's principal, i.e. the money put in by the owner is the owners money.

It makes no difference how old the annuitant (or owner) of the contract is, if they die then there is no penalty. Also, the Section 72 of the IRS Code states that the penalty is waived if the annuitant (or owner) is disabled. Generally, it must be the death or disability of the annuitant, not the contract owner or beneficiary, except where the contract is owner-driven, in which case all IRS penalties will be waived upon death or disability of the owner.

If the contract is annuitized, it will avoid penalty, but such annuitization must be elected by the contract owner within one year after investing in the annuity.

The age of the owner does not have to be 59 , indeed it is irrelevant.

The final way in which the 10 percent IRS penalty can be avoided is the contract owner being age 59 1/2 or older.

Because of these penalties, annuities are usually recommended for younger people unless it is part of a retirement plan such as an IRA or pension plan or profit-sharing plan. Of course, there is always the exception of the person who has sufficient funds so that they would not have to touch the funds in case of an emergency. Annuities are ideal candidates for the investor who is near or past age 591/2.

Unless the contract is owner-driven, the owner can be any age, from newborn to centenarian. But even with the penalty, it could still make good sense for a young person(s) but would depend upon how soon the money is withdrawn and the assumed rate of growth.

For instance, the owner of the annuity decides when withdrawals are to be made.
